Longitudinal course of endocannabinoids and N-acylethanolamines in hair of mothers and their children in the first year postpartum: investigating the relevance of maternal childhood maltreatment experiences

Background
Childhood maltreatment (CM) exerts long-lasting psychological and biological alterations in affected individuals and might also affect the endocannabinoid (eCB) system which modulates inflammation and the endocrine stress response. Here, we investigated the eCB system of women with and without CM and their infants using hair samples representing eCB levels accumulated during the last trimester of pregnancy and 10–12 months postpartum.
MethodsCM exposure was assessed with the Childhood Trauma Questionnaire. At both timepoints, 3 cm hair strands were collected from mothers and children (N = 170 resp. 150) to measure anandamide (AEA), 2/1-arachidonoylglycerol (2-AG/1-AG), stearoylethanolamide (SEA), oleoylethanolamide (OEA), and palmitoylethanolamide (PEA).
ResultsMaternal hair levels of 2-AG/1-AG increased and SEA levels decreased from late pregnancy to one year postpartum. Maternal CM was associated with lower SEA levels in late pregnancy, but not one year later. In the children's hair, levels of 2-AG/1-AG increased while levels of SEA, OEA, and PEA decreased from late pregnancy to one year later. Maternal CM was not consistently associated with the eCB levels measured in children's hair.
ConclusionsWe provide first evidence for longitudinal change in the eCB system of mothers and infants from pregnancy to one year later. While maternal CM influenced the maternal eCB system, we found no consistent intergenerational effects on early regulation of the eCB system in children. Longitudinal research on the importance of the eCB system for the course and immunoregulation of pregnancy as well as for the children's development.
United States response to the COVID-19 pandemic, January–November 2020

As of November 2020, the United States leads the world in confirmed coronavirus disease 2019 (COVID-19) cases and deaths. Over the past 10 months, the United States has experienced three peaks in new cases, with the most recent spike in November setting new records. Inaction and the lack of a scientifically informed, unified response have contributed to the sustained spread of COVID-19 in the United States. This paper describes major events and findings from the domestic response to COVID-19 from January to November 2020, including on preventing transmission, COVID-19 testing and contact tracing, ensuring sufficient physical infrastructure and healthcare workforce, paying for services, and governance. We further reflect on the public health response to-date and analyse the link between key policy decisions (e.g. closing, reopening) and COVID-19 cases in three states that are representative of the broader regions that have experienced spikes in cases. Finally, as we approach the winter months and undergo a change in national leadership, we highlight some considerations for the ongoing COVID-19 response and the broader United States healthcare system. These findings describe why the United States has failed to contain COVID-19 effectively to-date and can serve as a reference in the continued response to COVID-19 and future pandemics.

To promote the rights, well-being and development of the child, and for the benefit of families and the community, attachment should be a central focus of early childhood intervention (ECI) under the National Disability Insurance Scheme (NDIS). ECI Key Workers have the opportunity to positively influence parent–child relationships and are encouraged to do so by the ECI national guidelines. This article identifies how elements of the NDIS design and implementation may be counterproductive to fostering attachment security in children. These elements can lead to delayed intervention; increased parental stress; reduced expertise of service providers; and financial disincentives for best practice in working with disadvantaged families. The article highlights the implications for children with a disability and their families in Australian society and identifies lessons for the design and implementation of social policy.

With the advent of recreational sports like kite surfing and buggying, the performance of kites has become a market driven item. Producers increasingly require methods to measure and improve the performance of the kites they manufacture. The Mechanical Engineering Department at the University of Canterbury has been working with a local kite producer to develop testing procedures suitable for kite manufacturers. The primary performance measurement is the lift to drag ratio. An early test rig was mounted in the top of a car, but limitations inherent in the design meant that it lost precision as the lift to drag ratio approached that of more advanced kites. This led the investigators to look for alternatives, and resulted in the development of the circular flight method. This method allows the test apparatus to be tuned to the performance of each kite, significantly improving the precision of the results while reducing the time taken for each test. In their raw form, the L/D results are not quite the same as those of the more traditional methods. But they reflect the underlying aerodynamic characteristics, and when used comparatively they can be used in the kite development process. Alternatively, with suitable processing the circular flight results can be converted to the traditional forms.

A model with which to predict the effect of coplanar electrode geometry on diffraction uniformity in photorefractive polymer display devices was developed. Assumptions made in the standard use cases are no longer valid in the regions of extreme electric fields present in this type of device. Using electric-field induced second-harmonic generation through multiphoton microscopy, the physical response in regions of internal electric fields which fall outside the standard regimes of validity were probed. Adjustments to the standard model were made and the results of the new model corroborated through holographic four-wave mixing measurements.

Previously published guidelines are available that provide comprehensive recommendations for detecting and preventing healthcare-associated infections (HAIs). The intent of this document is to highlight practical recommendations in a concise format designed to assist acute care hospitals in implementing and prioritizing their central line-associated bloodstream infection (CLABSI) prevention efforts. This document updates “Strategies to Prevent Central Line-Associated Bloodstream Infections in Acute Care Hospitals,” published in 2008. This expert guidance document is sponsored by the Society for Healthcare Epidemiology of America (SHEA) and is the product of a collaborative effort led by SHEA, the Infectious Diseases Society of America (IDSA), the American Hospital Association (AHA), the Association for Professionals in Infection Control and Epidemiology (APIC), and The Joint Commission, with major contributions from representatives of a number of organizations and societies with content expertise. The list of endorsing and supporting organizations is presented in the introduction to the 2014 updates.

Background
Intake of soybean protein was associated with a reduced risk of breast cancer in a case–control study. It has also been demonstrated to increase menstrual cycle length in an experimental setting.

ResultsAn association (P=0.034) of higher intakes of soybean protein with increased menstrual cycle length, as recorded by self report and by prospectively recording three consecutive cycles, was observed. The risk of menstrual cycle length being greater than the median, when comparing the upper quartile (8.7–35.2 g day−1) of soybean intake and the lowest quartile (0.1–3.3 g day−1) was double, and this approached statistical significance (OR=2.02, 95% CI=0.88−4.64 and OR=1.93, 95% CI=0.82−4.56 for self-reported cycle length and cycle length as recorded by diary, respectively). In terms of the absolute association with cycle length, subjects in the upper quartile of soybean intake demonstrated a cycle length 1–2 days longer than did subjects in the lowest quartile.
ConclusionsIt is likely that the association between dietary intake of soybean protein and length of menstrual cycle prevails in the community setting. This is shown using both self-reported cycle length and cycle length as recorded in a prospective diary.
Does provider adherence to a treatment guideline change clinical outcomes for patients with bipolar disorder? Results from the Texas Medication Algorithm Project

Background. Despite increasing adoption of clinical practice guidelines in psychiatry, there is little measurement of provider implementation of these recommendations, and the resulting impact on clinical outcomes. The current study describes one effort to measure these relationships in a cohort of public sector out-patients with bipolar disorder.
Method. Participants were enrolled in the algorithm intervention of the Texas Medication Algorithm Project (TMAP). Study methods and the adherence scoring algorithm have been described elsewhere. The current paper addresses the relationships between patient characteristics, provider experience with the algorithm, provider adherence, and clinical outcomes. Measurement of provider adherence includes evaluation of visit frequency, medication choice and dosing, and response to patient symptoms. An exploratory composite ‘adherence by visit’ score was developed for these analyses.
Results. A total of 1948 visits from 141 subjects were evaluated, and utilized a two-stage declining effects model. Providers with more experience using the algorithm tended to adhere less to treatment recommendations. Few patient factors significantly impacted provider adherence. Increased adherence to algorithm recommendations was associated with larger decreases in overall psychiatric symptoms and depressive symptoms over time, but did not impact either immediate or long-term reductions in manic symptoms.
Conclusions. Greater provider adherence to treatment guideline recommendations was associated with greater reductions in depressive symptoms and overall psychiatric symptoms over time. Additional research is needed to refine measurement and to further clarify these relationships.

Conductive polymers such as polypyrrole (Ppy) are potentially useful as an active interface for altering cellular processes and function. Their utilization in medically related applications however have been substantially held back by their non-degradable nature. Herein we report a novel approach to creation of bioerodible polypyrroles via modification of pyrrole beta-carbon with an ionizable moiety. It has been shown that the erosion rate of acid-bearing derivative of polypyrrole increases with pH, which is consistent with the pH dependent ionization of carboxylic acid group. The novel paradigm proposed for the creation of bioerodible polypyrroles allows for simple and efficient control over the erosion rate of the substrate independent of the polymer chain length, via the choice of the terminal ionizable group and its concentration along the polymer backbone.

CdS nanoclusters ranging in diameter between 1 and 4 nm were prepared in aqueous solution using aliphatic mercaptoalcohols as ligands. The photon energies of the Is Is absorption and the respective oscillator strengths are in accordance with size quantization theory. Some clusters crystallize as macroscopic 3-D superlattices which were investigated by single crystal x-ray analysis. The neutral Cd17S4(RS)26 clusters are covalently linked in the superlattice the structure of which exhibits self similarity to the interior structure of the clusters.
Onion-shell-like composite particles from CdS and HgS were prepared by successive substitution and re-precipitation processes. Particles with a core radius of 2 nm, a shell of up to 1 nm HgS followed by a final shell of up to 1.5 nm CdS were obtained. Electrons and holes were localized in the HgS shell giving rise to excitonic fluorescence.

Ion irradiation and annealing studies were performed on Ni, Ni-20 at.%Al multilayers and Ni-20 at.%Al co-evaporated thin films. Xe+ ions were used to irradiate the films and homogenize the multilayers at room temperature. Irradiation of alloy films formed a metastable, supersaturated solid solution of γ phase and an HCP phase. Ion induced grain growth occurred in all films. A factor of 2 greater growth was observed in Ni-Al multilayers compared with coevaporated films irradiated to the same dose. The enhancement is attributed to a heat of mixing effect. Post irradiation annealing of the mixed multilayers formed γ*, the morphology of which was dependent upon the presence of Cu in the films due to substrate mixing from the support grid.
